ACCESS TO YOUR OWN MEDICAL RECORD
You have the right to access information we have collected about you, except in exceptional circumstances where access may be legitimately withheld. You will be given an explanation in these circumstances. A single A4 health summary sheet, to be approved by your doctor, will be made available free of charge. Further information, such as a full medical record, will incur a fee to cover the costs of resources and postage. Please ask reception if you would like access to your information, especially if changing surgeries or moving away from the area.

THIRD PARTY REQUESTS
We only transfer or release patient information to a third party once the consent to share information has been signed. Your patient rights to confidentiality continue even after death. This practice submits patient data to various disease specific registers, e.g. cervical, breast, bowel screening, to assist with preventative health management. You may opt in or opt out of this at any time.

PATIENT RIGHTS & FEEDBACK
If you are unhappy with any aspect of the care you receive at this practice, we need to know about it. Please talk to our Practice Manager about any problems you may be having. We recognise that patients have rights and we will endeavour to support those rights. 
We also have a suggestion box on the table in the waiting room if you would like to leave any comments regarding our practice. We believe that any feedback, good or bad, is best dealt with within the practice. However, if you feel your problem is not adequately addressed, you may prefer to write to Heath & Disability Services Complaints Office (HaDSCO) at GPO Box B61, PERTH WA 6001 or phone on 08 6551 7600 or 1800 813 583.
