ACCESS TO YOUR OWN MEDICAL RECORD

You have the right to access information we have collected about you, except in exceptional circumstances where access may be legitimately withheld. You will be given an explanation in these circumstances. A single A4 health summary sheet, to be approved by your doctor and will be made available free of charge. Further information may incur a fee. Please ask reception if you would like access to your information especially if changing surgeries or moving away from the area.

3RD PARTY REQUESTS

We only transfer or release patient information to a third party once the consent to share information has been signed and in specific cases informed patient consent has been sought. Your patients’ rights exist even after death.

This practice submits patient data to various disease specific registrars e.g. cervical, breast, bowel screening etc. to assist with preventative health management. You may opt in or opt out of this at any time.

PATIENT RIGHTS & FEEDBACK

If you are unhappy with any aspect of the care you receive at this practice, we need to know about it. Please talk to our Practice Manager about any problems you may be having.

We recognise that patients have certain rights and we will endeavour to support those rights. You also have the right to whether you wish to be contacted or recalled for any ongoing health care arrangements that we have in place. You have the right to opt out at any time by advising reception staff of your wishes.

We also have a suggestion box on the table in the waiting room if you would like to leave any comments regarding our practice.

We believe that any problems, good or bad with the practice are best dealt with, within the practice, however, if you feel your problem is not adequately addressed, you may prefer to write to ‘Heath & Disability Services Complaints Office (HaDSCO) at GPO Box B61, PERTH WA 6001 or phone on 08 6551 7600 or 1800 813 583.
